VOE Eligibility:
1) The school considers the student currently enrolled at the time the student applies for the VOE, and
2) the school awarded the student credit for each class the semester prior to application for the VOE.
If your student is at least 15 years old, they must follow these steps to request a Verification of Enrollment and Attendance Form (VOE):

Your student must request a VOE from their House Office at least 48 hours in advance.
The VOE verifies that the student met the 90% attendance requirement for the previous semester.
VOE Validity:
A VOE is valid for 30 days from the date it is issued.
Do not request a VOE if the student’s DPS appointment is more than 30 days away.
Signing the VOE:
Do not sign the VOE until you are at the DPS office in front of a DPS official.
Excused Absence for DPS Visit:
A student 15 years or older is allowed one excused absence to obtain a driver’s license or learner’s permit.
Bring back documentation from the DPS office to ensure the absence is coded correctly.
For further assistance, please contact your student’s House Office.
