Moving Request Form
Please use the Moving Request Form for moving items from one campus to another (i.e. boxes, choir risers).  Request should be sent to the Central Distribution Center with the following information:

• Date items are to be moved
• Date items are to be returned (if applicable)
• Complete description of request
• Location from and location to
• Contact person
